INITIAL VISIT:

On your initial visit, please be prepared with the following:

• A complete medication list, including prescriptions, over-the-counter drugs & vitamins
• A list of your consultant physicians and their addresses
• Your health insurance card
• A list of all surgeries and hospitalization dates
• Your address, phone number, social security number and emergency contacts
• A photo ID

For FAA Exams:
You will need to supply online confirmation number from FAA website at time of appointment. If you wear contact lenses, be prepared to remove and store your lenses for your exam.

For Workman’s Compensation Visits:
Your insurance will need to be verified with your employer, and you will need to know the date of the injury, the time it was first reported and the symptoms you first reported.
